my pt is terminal with heart failure. she was admitted to the hospital by hospice when her INR was 13. they did a CT scan because she fainted and fell. her ct showed bilateral subdural hematoma.... i need a nursing dianosis for this

He is drowsy and his heart rate is fairly fast and irregular...he is on insulin, so my first thought is he has very low glucose likely due to lack of food or too much insulin. Need to know his glucose level. I think the other info is just to reinforce that he is a very sick diabetic and perhaps to throw you off of the more urgent blood glucose level.

Wow, her INR is really high.
Sounds like this may have contributed to the bleed or it may have resulted from any injury sustained from the fall.

the nurse who over dosed the coumadin is going to pay the dying lady's family big bucks.
Dx: Nurses dont diagnose.

She has a bleeding and clot issue with an INR that elevated. She is at a high risk for a stroke. use something for bleeding/clot/stroke
